Stroke Schemes
Our Stroke Schemes offer people who have had a stroke the opportunity to meet on a weekly basis, and to receive practical and emotional support to support their rehabilitation and life after stroke. It also offers those who are looking after the stroke survivor, with an opportunity for respite for a few hours each week.
How does it work?
The Stroke Scheme Co-ordinator, in conjunction with volunteers and the individual will encourage participation in relevant structured activities, which can promote the physical well-being and improve the quality of life of stroke clients and their carers. There are a range of activities:
- a structured rolling programmes of therapeutic activity -including safe exercise aimed at your own ability
- help with transport and assistance to access the scheme
- goal setting and regular review of your progress
- advice and information on stroke and its prevention
- guidance and demonstrations to promote a healthy lifestyle
- Outings to places of interest - to help you adjust to your life after stroke
- Referral on and encouragement to use other services which can help you
- An opportunity to meet with other people who have also had a stroke
Therapeutic activities with physiotherapists, speech & language therapists and occupational therapists are organised throughout the year.
How can I be referred to a stroke scheme?
Any statutory key worker e.g. nurse, social worker or occupational therapist can refer you to the Stroke Scheme Co-ordinator who will contact you and carry out an assessment against criteria to join the scheme.
What will happen on my first visit?
The Stroke Scheme Co-ordinator will welcome you to the scheme and will work with you to design your own programme. We offer a two year period of attendance at the scheme.
