Weekly Stroke Services
Weekly Stroke Scheme
The Weekly Stroke Scheme offers stroke clients and their families and carers the opportunity to get involved in social and leisure pursuits, as well as education and employment through the Client Support Programme. This programme is tailored to the individual's needs with agreed achievable goals that are monitored on a regular basis.
How does it work?
The Stroke Scheme Co-ordinator, in conjunction with volunteers, carers and the individual will encourage participation in relevant structured activities. Such activities can promote the physical well-being and improve the quality of life of stroke clients and their carers. There are a range of activities:
- Health promotion and information
- Communication skills re-training
- Relaxation & stress management
- Arts & crafts
- Information on benefits and entitlements
- Social and community-based activities
- Games
- Visiting speakers
- Information for carers
Therapeutic activities with physiotherapists, speech & language therapists and occupational therapists are organised throughout the year.
How can I be referred to a stroke scheme?
Your statutory key worker i.e. nurse, social worker or occupational therapist can refer you to the Stroke Scheme Co-ordinator who will contact you. We aim to facilitate as many people as we can at the Scheme. In the instance that no spaces are available, your name will be placed in a waiting list. The Stroke Scheme Co-ordinator will stay in touch with you about your place.
What will happen on my first visit?
The Stroke Scheme Co-ordinator will welcome you to the scheme and will work with you to design your individual Client Support Programme. We offer a two year period of attendance at the scheme.
