Stroke Family Support
The Stroke Family Support Scheme aims to work alongside health professionals to provide a comprehensive support service for families of people who have had a stroke. This service is available for families as soon as possible after the stroke, providing them with information and emotional support. Stroke Family Support groups are organised within the five Health and Social Care Trust areas.
How does it work?
The service aims to make contact with families of people who have recently had a stroke, in hospital and in the community, within one week of referral. The service provides the following information and support:
- Information to help the family understand a stroke
- Healthy lifestyle information
- Acts as a 'link' between hospital and home
- Provides emotional support and assistance in adjusting to the changes caused by a stroke
- Referral to other support services e.g. local weekly stroke group
How can I be referred to a stroke family support group?
Hospital and community trusts staff can refer you to your local Stroke Family Support Co-ordinator. Families may also contact the Co-ordinator directly. Contact details of your local support group:
