"Taking Control"

 

What is the "Taking Control" Programme?

The "Taking Control" Self Management Programme is a structured group workshop:

  • Lasting 2.5 hours,
  • Once a week,
  • Over 6 weeks.

The programme aims to help people with long term health conditions develop the skills and confidence they need to better manage their condition day to day and beyond.

During each session participants look at ways to help manage the effects of their long-term condition such as:

  • Dealing with pain and tiredness
  • Dealing with feelings of frustration, depression and isolation
  • Learning relaxation techniques and increasing physical activity
  • Encouraging healthy eating
  • Communicating with family and health professionals
  • Reviewing appropriate use of medication
  • Planning for the future


Who can take part?

Anyone living with long term chest, heart or stroke related conditions.


How can the "Taking Control" Self Management Programme help me?

By taking part you will learn skills to help manage your health condition as well as developing confidence in yourself.

You will also meet other people who share similar experiences and understanding of living with a long term condition.
